Revocation of Consent to Electronic Notice and/or Voting

The undersigned Owner(s) of , who have previously consented in writing to electronic notice and/or electronic voting for meetings and elections of Pine Ridge Property Owners Association, Inc. (“Association”), hereby revoke my/our consent as follows:

Do you revoke your consent to receiving notice from the Association of Association meetings and other Association matters by e-mail? -  

Do you revoke your consent to vote electronically using the online voting system? -  

The undersigned understand and agree that if a Member is revoking consent to electronic voting, this form must be signed and on file with the Association no later than the deadline set by the Board of Directors prior to the meeting or election in which the Member wishes to revoke the consent to vote by electronic means, or the revocation will not be effective until the next membership meeting and/or election.

Furthermore, the undersigned understand and agree that if a Member is revoking consent to electronic notice, this form must be signed and on file with the Association no later than the deadline set by the Board of Directors prior to the Association sending notice of a meeting or election in which the Member wishes to revoke consent to electronic notice, or the revocation will not be effective until the next meeting and/or election.
